Elmwood Medical Centre Permanent - Full-time/Part-time/Portfolio opportunity£49,000 – £55,250 Advanced Clinical Practitioner / £56,500 - £65,250 Senior Advanced Clinical Practitioner (plus enhancements attached to KSF and dependent upon experience)Elmwood Medical Centre is situated in an idyllic rural area of Buxton, Derbyshire on the doorstep of the famous Pavilion gardens, we have an excellent opportunity for an Advanced Clinical Practitioner (Nurse, Pharmacist, Paramedic, AHP) to join the practice team.The practice has a list size of approximately#####0 patients and would involve collaboratively working within the wider high peak PCN, our care homes and our community services. You will be part of a busy and proactive team working alongside a multidisciplinary team including GP’s, ACP’s, Practice Nurse’s, Pharmacist, HCA and ARRS funding roles such as MSK & Mental health practitioners.Main duties of the jobAssess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.To lead and be proactive on quality improvement and service development.Networking with other clinicians and external stakeholders.Elmwood Medical Centre is part of gtd healthcare, one of the largest, not-for-profit, NHS commissioned primary and urgent care providers in North West England.
